In Voltaren Emulgel: the active ingredient, diclofenac diethylamine, penetrates into the skin and interferes with the effect of a substance called cyclooxygenase (COX) enzymes. COX-1 is important in pain and inflammation (swelling and redness). COX-2 is involved in inflammation (swelling and redness).
It reduces the production of prostaglandins (prostaglandins produced by the stomach or intestines). Prostaglandins are produced in the skin by the stomach and intestines, acting on them and making them ulcerating. When diclofenac diethylamine is applied to the skin, it penetrates into the connective tissue of the skin (prosthetic joint) which causes pain and swelling. When Voltaren Emulgel is applied to the skin, the drug penetrates into the skin causing pain and inflammation.
This action of diclofenac diethylamine on the gastrointestinal tract, on the skin and on the skin’s defence mechanisms may be responsible for reducing the effect of Voltaren Emulgel (see section 4.4).
